【緊急公告】特殊原因學生經防疫小組審核通過得以遠距方式教學
一、 配合教育部4月12日「校園因應COVID-19暫停實體課程實施原則」，各校應依個案足跡調查結果，滾動調整實體上課狀況。
二、 本校下列特殊情形同學經防疫小組審核通過，得進行遠距教學。
(一) 經衛生機構匡列之密切接觸者或本校衛保組通知離校自主健康管理者。
(二) 有肺部疾病或不適之證明者。
(三) 本人因健康因素無法施打二劑疫苗之證明者。
(四) 經校方通知離校自主健康管理期滿，因故須延長自主健康管理者。
(五) 境外生(含僑生)因疫情影響無法實體上課者；或因尚未取得健保資格，確診後承受自費醫療風險高者。
(六) 其他特殊狀況，請檢具證明文件，說明實體上課會有高風險者。
以上除第一款外，其餘皆須提出申請並提供證明文件，未成年者者需附家長同意書，並經防疫小組審核通過，始得以遠距教學方式進行。
三、 申請通過同學實驗/實作課程、實習課程及考試仍需實體授課，除非該教師同意可提供數位教學。
四、 若需申請同學請檢附學生因特殊原因申請遠距教學申請書及家長同意書後，E-mail至教務處信箱(acade@mail.cgu.edu.tw)提出申請，主旨請寫＂申請遠距教學＂，第一批申請截止日為4/21(星期四)。
五、 申請遠距教學並經防疫小組審核通過後，應儘速離校，以降低與其他校內師生接觸風險。

Students with special reasons are approved by the epidemic prevention team to be taught remotely.

1. In line with the Ministry of education's "implementation principles for the suspension of physical courses on campus in response to covid-19" on April 12, all schools should adjust the physical classes in a rolling manner according to the results of the case footprint survey.
2.The following special circumstances of our school have been reviewed and approved by the epidemic prevention team, and they are allowed to conduct distance teaching.
2.1 The close contacts listed by the health institution or the school's health security team will be notified to the school leaver's independent health manager.
2.2 Proof of lung disease or discomfort.
2.3 Proof that I am unable to receive two doses of the vaccine due to health reasons.
2.4 Upon the notice of the school, the independent health manager shall be extended for some reason after the expiration of the independent health management period.

2.6 For other special circumstances, please check the supporting documents, indicating that there will be high-risk persons in the physical class.
Except for the first paragraph, all the others must submit an application and provide supporting documents. For minors, a parental consent form must be attached, which must be reviewed and approved by the epidemic prevention team before remote teaching can be carried out.
3. Physical teaching is still required for students who apply for passing classmates' lab/practice courses, practicum courses and exams, unless the teacher agrees to provide digital teaching.
4. For students who need to apply, please submit the application form for distance learning due to special reasons and the parental consent form, and then E-mail to the mailbox of the Academic Affairs Office (acade@mail.cgu.edu.tw) to apply.
